NEW PATIENTS (Adult Diabetes)
As a new patient, we know you have many questions! Many of these questions will be answered during your appointments with our doctors and staff!

WHAT TO EXPECT FROM YOUR FIRST VISIT
As a new patient there will be TWO SEPARATE APPOINTMENTS:
1. The Pre-Assessment Appointment
This visit will last approximately 60 to 90 minutes. At this time, Diabetes Educator will do a complete medical history, provide instruction on monitoring blood sugars and order additional laboratory testing if required.
It is our policy that if this first part of the appointment is NOT kept, the physician appointment will be canceled. This meeting is very important!
Please bring the following for your pre-assessment appointment:
+All of the Patient Information sheets and forms with the authorizations for treatment signeD.
+The Medical History Records
+ALL of the actual prescription bottles of any medications you are currently taking
+Your blood glucose meter and supplies
+Your log book with recorded blood sugars
+Your Insurance Card
2. The Physician Appointment
The physician will do a complete physical examination, discuss your diabetes and explain any test results available. They will discuss individualized program of medication, diet and exercise. Additional laboratory tests may be ordered at this time. This visit will take up to one hour.

You child has been referred to EDCC for an evaluation and consultation of diabetes. At your first visit, the doctor will spend at least one hour going over the condition with you. He or she will ask you about your child’s history and medical background. The following medical records may be very helpful; please bring them with you:

+Any Hospital Summaries you may have
+Previous Laboratory Tests

Your child’s scheduled time is one hour and every minute of that hour is important as we discuss the plan for treating and living with your child’s diabetes. DROP-INS
The Endocrine & Diabetes Care Center cannot accommodate new drop-in patients. All new patients must be referred to the EDCC from a Primary Care Physician.
